Why anyone buying podiatry practices is in it for a short sale, not long the long term.
There are less and less students applying to the podiatry colleges every year. This of course means there will be less graduates over the next years. This means there will be unfilled residency programs, which will eventually close. That means less people will be finishing residencies. Maybe some of them will continue to attend fellowships or what we call a fellowship in podiatry. We will have fewer podiatrists. How does this impact private practice podiatry ?
The answer is straightforward. Those completing the expensive journey to become a podiatrist will be getting the good jobs with hospitals, healthcare groups, orthopedic groups, and will be practicing their dream podiatry. They will be doing surgery and advanced care and will not be bothered with so called podiatry “at risk care” a dignified term for treating corns and calluses and toenails.
Private practice will start to die out. Initially, those remaining in private practice would likely be busy. However, as they retire and die out, there would be very few private practice podiatrists available. Perhaps there will be a conversion to a cash fee for service similar to dentistry and veterinary medicine. If those podiatrist do a good job and satisfy their patients/customers, they will likely do very well. There will be fewer podiatrists, more people seeking their care, and they can likely charge for providing good chiropody and podiatry services.
Overall, however, as podiatrists become employed, there will be less access for comfort care, and palliative care. People will seek this out from alternative healthcare providers. Over time this will decrease flow to the offices of the remaining private practice podiatrists.
Another factor to look at is the growth of the American Orthopedic Foot and Ankle Society. Members will remain very busy and so long as the number of patients overwhelm the availability of qualified orthopedic surgeons, podiatrists will continue to pick off surgical procedures. Podiatry will succeed in small towns and smaller communities. As healthcare consolidates and referrals consolidate, a podiatrist will either have to be part of the system, or will have to struggle to make a living, hustling orthotics, selling useless products and therapies, and talking people into surgeries that are borderline not needed.
So yes, podiatry will survive for at least another 10–15 years slowly deteriorating like a patient who has been told they have terminal cancer that will be surviving for another five years before their ultimate demise. Like many of the cancer drugs, the process might be slowed down, but the end result will be inevitable. It is a “specialty” that just does not belong in the future of medicine. The survivors will be employed podiatrists. Private practice will be dead.
Take the money and run. That is the new podiatry mantra.
I honestly believe the discussions on foot care nurses have brought PM News and it’s “contributors” to a new low. This is something not thought possible. Fighting over toenail trimming. 8. I am tired hypocrisy in podiatry! One guy writes in how terrible other people like , nurse do our work. Why not? It would be cheaper! This particular podiatrist collected 2 years ago according to Medicare data 150,000 in home visits while cutting toenails.Another complaints that denial of expensive skin substitutes. He collected 198,000 from Medicare for this.
To deny that podiatry is a dirty profession is to ignore it’s lifers and controllers. Of course. But even if an MD, DO or RN can treat the whole body, earn a wonderful living,
have greater job opportunities, they likely do not qualify for the opportunity DPMs have to use the money they have to pay dues money to qualify to join the APMA and state podiatric societies. Remember that even if a DPM has no money for dues from their practice, they still have the opportunity to get money from other sources outside of the podiatric profession.

If RNs and PAs get additional training to treat the feet my opinion is the public benefits from additional access. However, a DPM degree is needed to qualify for the privilege paying dues to qualify for membership in the APMA.
Perhaps the APMA bylaws can be changed to allow RNs and PAs to spend their money to qualify for membership in the APMA.

Most podiatrists are honest and care about helping others. The APMA is helpful in making people aware about podiatry. Think of the smile on your patients face when you cut out a painful mycotic toenail. Patients able to walk without a painful ingrown toenail improves the quality of their lives.
