Podiatry Logical Conclusions

Yes another great debate in PM News.  Should one correct a painless bunion deformity ?
Elliot the Idiot of course chimes in with another effort that misses the toilet bowl and puddles on the floor. Forget that for a minute. So people write in and draw an analogy between a painless bunion and a painless malignancy.  Brilliant.
They argue we should correct painless bunions because eventually they will cause all sorts of horrible problems.
Of course there is no literature to demonstrate this is true. But let us say that this is somehow the argument. What does podiatry logic tell us? Well, given the high incidence of recurrence of bunion deformities we are compelled to do a Lapidus procedure.
OK, now we have a painless bunion undergoing a Lapidus procedure. But wait. These bunions occur due to pronation deformity.
And we of course have literature telling us that it is improper to just correct the bunion as this is a symptom of a more global process.
So now we must do some type of pronation limiting surgery, at a minimum sticking some crap in the subtalar joint.
So now we have a painless bunion undergoing a Lapidus (laugh at us) and of course pronation limiting surgery. I suppose the next step is to argue that we must be doing surgery on non-symptomatic flexible flat feet in children as well as adults.
After all, flat feet can lead to all sorts of horrible problems such as plantar fasciitis and hammertoes.
Why stop with just selling children useless unproven devices endorsed by the so-called pediatric arm of the APMA? This is of course podiatry logic to the maximum. We have some idiot writing in who cannot figure out why a foot may be swollen and red and warm with changes suggestive of arthritis in the great toe joint.
Now we want these people making decisions on surgery for non-symptomatic deformities.
This is what happens when you train people and turn them loose without a population requiring the services that they have been trained to provide.
Now we are looking at treatment of non-symptomatic deformities with no evidence, no studies to suggest these bunions will result in any significant problems in the future.
After all, we would not want to wait until some of those people might develop problems to correct those at a later date. This will not pay the bills that we have to pay right now. One last thought. Who in their right mind would allow Elliot the Idiot to perform any type of bone surgery on their foot ?
He points out that orthodox Jewish people would not allow him to correct deformities.  I say this is a tribute to their religion.
There are some people who believe that Jewish people are inherently smarter. This might be used as a good example to verify that point.

Greg, perhaps you went to one of the 200,000 (out of a million bad MD’s). That leaves you 800,000 good ones to look at your throat. Why didn’t you go to a SPECIALIST INSTEAD OF A PCP? Are there any PODIATRIC OTOLAYRNGOLOGISTS? BTW, Think of all the differential diagnoses that the MD has to R/O and that the MD is HELD ACCOUNTABLE for! From Barrett’s Esophagus and reflux to Strep to HIV! Good or bad dx., THE MD IS STILL ACCOUNTABLE for ALL! Compare that to the few minor differentials wtf’s have to make.

I didn’t go to a specialist, thinking the PCP is where I should start. And, quite frankly, my copay is lower. I’ve had strep throat and the presentation is different, let alone it feeling like a knife was slitting my throat when I swallowed. Barrett’s or HIV? Zebras. I look for horses. My opinion, but I feel oral (esophageal) thrush should be pretty high on a PCP’s DDx list. Even dentists are trained for this.

Yep, precisely. A while back I went to my PCP with esophageal thrush from my nasal steroids. LPN takes my vitals, asks why I’m there, then comes the PCP. Looks in my throat, said it was post nasal drip. I had some pictures I took with my cheap endo camera just for shits, and of course they were dismissed. I’m not sure what your point is. Are you comparing MD fraud to wtf fraud to show that wtf’s are “just like” or wtf’s are more pure ? The 2018 census says there are about 1million MD’s and about 13,000 wtf’s. No one is saying MD’s are immaculate. I said it would be an interesting study to compare the %. Block seems to have at least a wtf/month committing fraud. In a field that is only about 1% the size of MD’s its something to think about. Say that 200,000! But this doctor is an MD and knows how to pick up cardiac abnormalities and can recite the Krebs cycle backwards AND is an expert in electrolyte abnormalities. Despite the FACT that he wouldn’t recognize a stethoscope since he hasn’t seen one in 30 years, etc., etc. But he IS an MD and would NEVER do anything that violated the Hippocratic Oath. I’m sure he took that $400,000 in inappropriate fees and donated it to some charity. After all, he is an MD and entered the profession to be altruistic.

The suggestion that pronation surgery is required with bunion correction is short-sighted. What if the patient has scoliosis causing a functional limb length deformity resulting in the foot pronating on the longer leg ? Should we not correct that deformity ? Of course. My question: if the scoliosis is the etiology of the bunion, can we correct the scoliosis ?
