For Podiatrists the 2021 E and M changes are not that big of a deal

So, the latest crazy, the sky is falling talk on the lecture circuit is the coming changes to E and M codes for Podiatrists and regular doctors alike. These codes were shepherded in by the AMA. But like a lot of things these codes are most likely not nearly as big a deal as Podiatrists are making them out to be. Here is an article that discusses precisely why these changes to the Evaluation and Management Codes for Podiatrists and other doctors alike is really not a big deal.

If one can earn a 4 year PA degree, make 120K gross-32-40h work week, practice full scope with lotsa options, be truly needed, tons of job ops, and graduate with little/much less debt, why the f would anyone with a working brain stem sign up for the p nonsense? Could someone maybe give a cogent argument for this tomfoolery? Most in DMP land get their money from NOT doing podiatry

The glamor of the “doctor” title is the lure. In the old days that title meant something. Now you’re providers and everyone and his brother has some kind of “dr” title. The foot tech with a dr title looks down on the PA because he’s not called DOCTOR even though the PA in actuality is much more of a DOCTOR than a wtf is. Nevertheless, its good socially and makes the medical school reject feel better about himself. That undeserved title keeps foot schools open but people are catching on.

Podiatry’s “reputation”? Smelly feet, bunions, diabetic salvations/woundacology, toenails, and shoes/orthotics–all for a $. That’s really it. A 3rd chance to do something somewhat health care related and be called “doctor” to save face to family and friends. The ‘smart’ ones are the folks NOT doing podiatry. They sell podiatry, classes, fake “experts”, CON sulting, courses, techniques, coding shysters, ads, marketing fads, etc..etc…as they laugh themselves to the bank.

ooPS: HIT SEND BY MISTAKE. I was saying a few pods took the DC route to try to get some scope that the failed podiatry profession couldn’t. They (wtfs) all dance around trying to emulate the PA or MD. Podiatry failed ,so people find TRICKS to look more relevant. I’ll bet he cracks backs more than toenails. Oh how great it is to be elected “CHIEF OF PODIATRY”. Man who cares except a narcissit that needs approbation. Chief of podiatry is like chief of the janitors. Who cares except WEAK podiatrists. And, what’s with all the talk about RUDE PATIENTS. Maybe they’re rude because the non doctor is crude? And what do you call DUSTIN RIDDLE. HE WENT TO CHIROPRACTIC school, he went to foot school and he got an MS in administration. Mix them all together and it doesn’t equal a P.A. in scope, need or mainstream acceptance. A few pods did the D.C.

Why those DIRTY, FILTHY, CROOKED, PODIATRISTS!! Thank You to “do you trust your podiatrist” for the link to the ACFAS TAX FORM. I bypassed it before but checked it today. It’s worth the quick review. The ACFAS is worth approx. 18 MILLION DOLLARS from their EXTORTION BUSINESS and the salaries are CRAZY to say nothing about their expense accounts. Podiatrists with dermatoscopes have greater knowledge of structures and patterns beneath the skin
that dermatologists without dermatoscopes do not have such instrumentation usually leads to better more informed decisions on when and where to biopsy.
Furthermore, name one podiatrist board certified on foot surgery or a dermatologist that removed the osseous structures from gangrenous toes resulting in healing and soft tissue regrowth as per http://www.journalofdermoscopy.com

NO PODIATIST, boarded or unboarded, ever “removed osseous structures from gangrenous toes resulting in regrowth of toes”. You ‘THINK’ YOU DID, BUT YOU misinterpreted granulation tissue for normal regrowth. Doing a biopsy is the “standard of care” dermoscopy is not. Dermoscopy is qualitative and NOT DOING A BIOPSY IS MALPRACTICE. No court would excuse you if you said “I did dermoscopy and didn’t think a biopsy was necessary”. Biopsy alone is not the last stage of evolution. Please see atlas of dermoscopy 2nd edition page 203. Radial growth invisible to the naked eye such as pseudopods does exist. My guess is the word has spread and many podiatrists are aware that a verticle biopsy might miss such structures and patterns. I learned years ago that this is not a contest as to what is better a biopsy or dermatoscopic exam. Biopys alone is not always as accurate as biopsy combined with dermoscopy. Shlock’s rag was the CNN/PBS/NPR/MSM of podlietry. And YES-Robert the propaganda “CNN” “news” stating that President Trump symptoms were severe, grave, horrible, etc.etc. then the cognitive dissonance must be outstanding as he gives talks from Ward 71 and riding for his fans. This is not a contest of who knows more about dermoscopy.
All I care about is that MD’s, DPM’s and others embrace the use of dermoscopy to be better informed to help make better decisions regarding podiatric care.
Additionally, this is also not a contest which is more important a verticle biopsy vs a dermatoscopic image. Both are important.

Anyway, I just read where John Wayne Bobbitt (of penis decapitation fame) stepped on a nail while doing construction work in my beloved BUFFALO, NY. The article said he went to two hospitals and saw several doctors and he has a “bone infection”. They may have to amputate his foot. I wonder if he saw one of ‘THE’ specialist of the foot or just some orthopedic surgeon MD who never learned biomechanics and therefore knows nothing about the foot.
