Need help understanding the naming of chemical structure of carboxylic acid vs carbonic acid. The importance of this is when too much carbon dioxide is in water there is a change to carbonic acid and this could then dissociate to have h+. Thus the acid base balance in blood coul be affected. A podiatrist could keep current. This is why I purchased not only Guyton but also Lippincott, and Boran texts All physiology texts So as to keep current and get different opinions. Once a week for many years I have conversed with many physicians, discussed patient cases. A DPM could be competent ordering lab tests as a physical exam for podiatric symptoms. This is far better than going to a foreign medical school and not getting matched with residency training and not being able to practice medicine in the US. This is why it’s the practice of podiatry. Ordering tests for podiatric symptoms and asking other physicians for input on what to order. Do this for many years and one could get very adept at it. Even if you do not factor for variables (like English as a second language, being a U.S. citizen etc.) and if you accepted the NON CORRECTED 30% rejection rate, that still means you have a 70% chance of a REAL U.S. ACGME residency. For example edematous feet may be present and a podiatrist orders labs to rule out kidney pathology as a cause. If a renal impairment is revealed a podiatrist can refer the patient to a nephrologist and for a return visit to monitor the foot edema
As well as for future monitoring by podiatrist.

I remember During freshman year at NYCPMA in 1980 we were told to purchase an ophthalmoscope taught about the red reflex and examining vascular structures.
However I personally prefer a dermatoscope
Regarding examining vasculitis.
Toenail vascular structures also can be examined to rule out vascular structures
When dealing with a podiatric foot sign or symptom of a positive collagen vascular disease or even possibly a pedal manifestation of COVID.

Does any person reading this blog agree that pods and MD’s are similar regarding scope to perform physical evaluations? If MD’s have the same scope as wtfs, why do wtf’s need to exist? If you have a pt with Bilat. edema can you order an echocardiogram or BNP? If you see black spots in the nail can you order a D DIMER ? A DPM could basically think that broadly especially since the Internet exists and the DPM could be part of a team guiding and training them on what to order. This is why it’s the practice of podiatry. Ordering tests for podiatric symptoms and asking other physicians for input on what to order.
Do this for many years and one could get very adept at it. Podiatrists should embrace the future and this is training and learning from others and experience while in practice to increase ones skill set. As a podiatrist, am blessed to be part of mainstream medical care.
