I absolutely disagree with the conclusions regarding the etiology of this problem. I honestly do not think in most instances it is lack of access. It is hypertension, poorly controlled diabetes, smoking, extremely poor diet choices, and noncompliance. No one wants to admit the truth. With all the advances in vascular surgery and cardiology, with all the “wound care centers and certification“, with all of the wound care experts, with all the corporations making wonderful and fantastic grafts, the advanced dressings and wound care products, the amputation rate is going UP not down. That’s because PODIATRISTS DON’T REALLY DO LIMB SALVAGE. STENTING, REVASCULARIZATION IS LIMB SALVAGE, NOT PUTING A DRESSING ON A WOUND AND BILLING THE HELL OUT OF IT.
